Turn project risks into visible actions
A standalone risk register tends to be written once, at the start of the project, and opened again only when something has already gone wrong. The reason is structural: if the register is not part of the reporting a manager uses weekly, there is no moment at which it has to be reviewed.

Assess probability and impact
Each risk carries a probability and an impact rating, giving a comparable exposure figure so the register can be ordered by significance rather than by the date it was entered.
Assign responsibility
Every risk has a named owner and a next action. A risk without an owner is an observation; the register is structured so that distinction is obvious at a glance.
Monitor mitigation
Mitigation is tracked as status and date, not as narrative text. Overdue mitigation actions surface in the action tracker that leadership already reviews.
